Licensed Practical Nurses at VNH are an integral part of the care team, where they assess a patient’s health status and complete nursing interventions in collaboration with the interdisciplinary team members and the patient.

The LPN will:

• Provide nursing services to individuals and families in their residence under the supervision of a registered nurse.


• Perform prescribed individualized patient care to patients as delegated.


• Assist the RN in the preparation, evaluation, and modification of the patient's plan of care as appropriate.


• Observe, document, and report to the RN as appropriate, the patient's physical and emotional needs and reactions to care.

Minimum Qualifications:

• Must be a Licensed Practical Nurse in Vermont and New Hampshire or hold an unencumbered compact license.


• A graduate from an approved school of practical nursing.


• 2 years experience in home health setting.
