• Primary Worksite – 1200 S Columbia Road (Clinic and Hospital)

• Worksite Setting (clinic/hospital, inpatient/outpatient/both, # of beds) –

• Work Schedule (Dates of coverage, schedules/hours, call schedule) – M-F 7:00am-4:30PM

• Patient/Caseload Info. (Patient per day/hour, call volume, procedure volume) – Rounding on inpatients and seeing clinic patients (10-14 clinic patients per day)

• Staff Info. (Support staff info.) – There is an RN and LPN to support in clinic, two CT surgeons and 2 APP’s (one on leave)

Provider Requirements:

• Board Certified –

• Required Procedures/Skills (Need details): Must have previous experience in cardiothoracic surgery.
