Key Responsibilities:

• Perform routine patient assessments and monitor changes in condition

• Administer medications and treatments as prescribed

• Assist with wound care, dressing changes, and basic IV therapy (where permitted)

• Support activities of daily living (ADLs), including mobility and personal care

• Accurately document patient care in EMR and maintain updated records

• Take and record vital signs, intake and output, and report abnormal findings

• Communicate effectively with the healthcare team to support care plans

• Educate patients and their families on care needs and safety precautions

• Respond to patient needs promptly and provide compassionate support

• Follow infection control, safety standards, and facility policies

• Participate in admissions, transfers, and discharges as needed

• Maintain confidentiality and comply with HIPAA regulations
