Job Summary
We are currently hiring a qualified Physical Therapist (PT) for an immediate Contract opening (Day (5x8) shift) in Sonora, CA. In this position, the professional will provide customized home-based physical therapy evaluations and rehabilitative treatments to improve mobility and functional independence in the home setting. Primary responsibilities include: perform in-home mobility, gait, balance, and fall-risk assessments for homebound patients, develop and deliver home exercise programs, gait training, and transfer techniques, and recommend home safety modifications and adaptive mobility equipment. Candidates must possess valid credentials/licensure, relevant clinical background, and a strong commitment to quality patient care.
