- SF valley West Hills, North Hollywood, Glendale, Burbank, Northridge, Sherman Oaks
- Glendale/Burbank/Tarzana/Pacoima/North/Hollywood/Northridge
- May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address.
- Average 4-6 visits per day depending on type of visit for PT/OT/SLP
- Productivity is 28-32 points per week for PT/OT/SLP
- Home Health and Oasis experience required, no exceptions
- Homecare Homebase documentation experience required
- Client may consider without HCHB exp at a reduced rate for the first 2 weeks if the manager is willing to train.
- 2 weeks cancellation
- Any overtime must be pre-approved and requires written approval from Administrator
