Registered Nurse (RN) - Home Health

We are currently hiring a qualified Registered Nurse (RN) - Home Health for an immediate Contract opening (Flexible shift) in Grand Junction, CO. In this position, the professional will deliver individualized home-based skilled nursing care, patient assessments, and chronic disease education for patients in home care settings. Primary responsibilities include: travel to patient residences to perform clinical assessments, wound care, and IV infusions, develop, update, and implement individualized home health care plans under physician direction, and educate patients and caregivers on medication management, fall prevention, and self-care techniques. Candidates must possess valid credentials/licensure, relevant clinical background, and a strong commitment to quality patient care.
